We want to do everything we can to eliminate any unnecessary stressors, which is why we include the option of scheduling your appointment and completing your forms online. Using the link below, schedule to receive a New Patient Initial Exam (New patients only). After you schedule your appointment, you will receive an email with a link to complete your intake forms online. If you are unable to complete the forms online, please arrive for your appointment 10-15 minutes early to complete them in our office.
What to expect
Your Initial Exam will include a thorough physical examination and consultation, including an explanation of your diagnosis and treatment options. You will then receive Premium Chiropractic Care which may include spinal adjustments, extremity adjustments, soft tissue treatments, physiotherapies, mechanical flexion distraction, and/ or home exercises/ instructions.
